Southwest Florida Pain Center is dedicated to providing our patients with quality healthcare at affordable prices. For your convenience we offer three options that you can use to pay for your services. Select the option below that works best for you. Feel free to speak with a patient care advisor for more information, to verify coverage, or to get help filing insurance claims.
• OPTION 1 • Easy Self Pay
This option works best for patients who do not currently have health insurance, have very high deductibles on their health insurance or have already met their flex spending limits for the policy year.
How it works:
For a one-time fee of $650.00 you will be provided with your initial office visit for diagnosis, the recommended in-office procedure* and one follow up appointment.
* Types of in-office procedures will vary by patient. Please consult with your provider for the procedures covered with this option.
Additional procedures may be proposed as part of your treatment plan. Please discuss with a patient care advisor regarding the self pay fee for additional procedures.
• OPTION 2 • Health Insurance
We currently accept most insurance companies such as:
• Blue Cross Blue Shield
• United Health Care
• Aetna
• Cigna
• Medicare
If you don’t see your company listed above, please consult with your patient care advisor when making your appointment.
Any deductible payments or co-pays for your appointment will be due at the time services are rendered. Southwest Florida Pain Center does not currently offer payment plans on balances for services rendered. If you do not currently have health insurance, please refer to options 1 and 3 for alternate payment methods.
• OPTION 3 • CareCredit Financing
For your convenience Southwest Florida Pain Center now accepts CareCreditSM as a payment option for our services.
Whether you need to cover your deductible or pay for treatments and procedures not covered by insurance*, CareCreditSM helps make the health, wellness, and treatments you want possible today*. Simply use the estimator and application features below to see payment options and apply. Full rates and credit card agreement terms are available before you apply.

CareCreditSM is different from a regular credit card.
Use it to pay for out-of-pocket expenses not covered by medical insurance, and special financing options are available that you may not be able to get with other cards.
Pay for health, and wellness care at over 200,000 enrolled providers across the nation. Once you've applied, you can use it again and again at any location that accepts CareCreditSM .
With shorter term financing options of 6, 12, 18 or 24 months no interest is charged on purchases of $200 or more when you make the minimum monthly payments and pay the full amount due by the end of the promotional period. If you do not, interest is charged from the original purchase date.*

CareCreditSM also extends longer term healthcare financing for 24, 36, 48 or 60-month periods with Reduced APR and Fixed Monthly Payments Required Until Paid in Full. Purchases of $1,000 or more may be eligible for a 24 months offer with a 14.90% APR, a 36 months offer with a 15.90% APR or a 48 months offer with a 16.90% APR. Purchases of $2,500 or more may be eligible for a 60 months offer with a 17.90% APR.**

Ask your provider to find out which promotion is right for you. You do not need to select a promotion before you apply.

Additional policies of Southwest Florida Pain Center:
1 We make every attempt to accommodate scheduling based on the needs of our patients. At certain times our availability is limited due to high patient volumes. If you have an appointment, we require 24 hour or more notice if you need to cancel your appointment. Any appointment cancelled less than 24 hours from appointment time will be subject to a $50.00 cancellation fee.
2 If you do not cancel your appointment or are unable to come at your appointment time, there will be a $50.00 fee charged for not cancelling your appointment.
3 Any amounts due by the patient after all insurances are applied is due at the time services are rendered. Care CreditSM cards can be used to pay the remainder of any balances owed to Southwest Florida Pain Center. All major credit cards are also accepted.
4 Southwest Florida Pain Center does accept checks as payment for services rendered. There will be a $10.00 fee assessed for all returned checks.
